did you write a birth plan? What should I put in it? We are planning a free-standing birth center birth, but I'm worried about what could happen during a hospital transfer. I'm terrified of c section unless it is the only way to keep baby and mom safe.

No I didn't write a birth plan. I just figured that whatever needs to be done will be done (not a very good plan, I know). Try not to worry too much about the negatives, focus on the good stuff.
