Do you use a V-Chip? It basically blocks shows with a rating not approved for children. I personally don't, but my neighbors are complete sticklers. They have anything above 7 or older blocked.
You should be aware of what your children are watching. You don't really need some kind of device to monitor your child's television viewing, you should watch with them and make sure they know what programming is acceptable and what you don't want him/her watching. Better to teach them than to restrict them.

I had a tv with the v-chip, but never used it. If you watch tv with the kids then tv is being monitored by the parent. The thing is, if your child goes to another's home, friend or relative, you can't always be sure that the tv there is being monitored.

It couldn't hurt. A little extra protection for when you have to leave the room for a minute. Children can be curious. But Seeker is right, you're not going to have any control over what they might watch at a friend's house. So you should make your opinions clear to them even if you do use a v-chip.
I don't really like shows like South Park and such, so I don't watch them. I definitely don't let my siblings watch them either.
